Providence Social Worker BSW in Missoula, Montana
Providence is calling a Social Worker (Social Services, Days, 0.6 FTE) to Providence St. Patrick Hospital in Missoula, Montana. In this position you will:
Assess patient/family learning needs, abilities, preferences and readiness to learn. Complete on going assessments and documents according to policy.
Assess, collaborate and coordinate patient’s discharge planning needs and readiness for discharge with other members of the care team.
Communicate with other members of the health care team regarding the patient’s needs, plan and response to care.
Consistently receive and give suggestions and constructive criticism in a professional manner.
Consult regarding cases that are presenting problems and/or experiencing significant deviations from the plan of care, conferring with the Medical Director/Department Director as needed.
Demonstrate strong interpersonal skills and ability to communicate effectively with diverse populations, including employees, physicians, patients/families and external agencies.
Screen, assess, monitor, document and interpret psychosocial, spiritual, environmental, self-care, educational, and discharge planning needs.
In collaboration with the patient/family, and in a developmentally and culturally appropriate manner, implement the plan of care according to established Social Work policy.
Maintain current knowledge of reimbursement and regulatory issues of the payors most frequently seen with the patient population.
Obtain a history through the interview process, maintains ongoing monitoring/assessment of assigned patients.
Participate in the development of patient plan of care to include patient problems, goals, evaluations and interventions.
Report symptoms and significant changes in patient’s condition to appropriate personnel.
Take initiative in assisting other members of care delivery team.
Use age specific standards, policies and procedures and guidelines to perform social work tasks that are routine for unit/clinic patients.
Utilize appropriate teaching methods and/or referrals to ensure patient/family educational needs are met. Provides community resource information to patient/family and evaluates the effectiveness of teaching, making adjustments as necessary. Documents education process on appropriate screen/format.
Utilize available information to evaluate patient/family progress toward goal attainment and makes adjustments in care as indicated.
Work with health care team and outside resources to facilitate optimum patient care delivery.
Required qualification for this position include:
- Bachelor's degree in Social Work
Preferred qualification for this position include:
- 2 years prior Medical social work
