Providence Health & Services Social Worker BSW in Missoula, Montana

Description:

Providence is calling a Social Worker (Care Coordination, Day Shift, 1.0 FTE) to Providence Medical Group in Missoula, Montana. We are a community of caregivers delivering every day on our Mission to provide compassionate care that is accessible for all – especially those who are poor and vulnerable.

In this position you will:

  • Assess patient/family learning needs, abilities, preferences and readiness to learn

  • Complete on going assessments and documents according to policy

  • Communicate with other members of the health care team regarding the patient’s needs, plan and response to care

  • Evaluate response to plan and make adjustments as indicated

  • Implement the plan of care according to established Social Work policy in collaboration with the patient/family, and in a developmentally and culturally appropriate manner

  • Obtain a history through the interview process, maintain ongoing monitoring/assessment of assigned patients

  • Participate in the development of patient plan of care to include patient problems, goals, evaluations and interventions

  • Report symptoms and significant changes in patient’s condition to appropriate personnel

  • Utilize appropriate teaching methods and/or referrals to ensure patient/family educational needs are met

  • Provide community resource information to patient/family and evaluate the effectiveness of teaching, making adjustments as necessary

  • Document education process on appropriate screen/format

  • Work with health care team and outside resources to facilitate optimum patient care delivery

Qualifications:

Required qualifications for this position include:

  • Bachelor's degree in Social Work

Preferred qualifications for this position include:

  • Minimum 3 years Social Work experience

  • Strong professional level of knowledge and comprehensive clinical assessment skills in the adult population and chronic disease management
