Providence RN - Care Manager in Missoula, Montana
Providence is calling a Registered Nurse (RN) Care Manager (Internal Medicine, Days, 1.0 FTE) to Providence Medical Group Montana in Missoula, Montana. In this position you will:
Assess barriers to care and stressors.
Assess the ongoing care needs of patients through interaction with patients, families and members of the health care team. Coordinates and plans for care consulting interdisciplinary team members as needed.
Enhance communication and collaborative relationships with multidisciplinary healthcare team members.
Establish a plan of care with patients, providers, and payers while continuously reassessing cost-efficient, appropriate levels of care.
Emphasize continuity of care, thus reducing or eliminating fragmentation, duplication or gaps in treatment plan. Assists with any transition in care.
Provide appropriate evidence based patient education, monitors health needs, and coordinates community resources based on patient care needs.
Prevent adverse patients occurrences when possible and intervenes quickly if prevention is not possible, thereby minimizing poor outcomes.
Required qualifications for this position include:
Associate's Degree Graduate of an accredited school of nursing.
Current licensure in the State of Montana.
Current AHA BLS
Strong professional level of knowledge and comprehensive clinical assessment skills in the adult population and chronic disease management.
Preferred qualifications for this position include:
Bachelor's Degree Graduate of an accredited school of nursing.
