Providence is calling a Registered Nurse (RN) Care Coordinator (Anticoagulation Clinic) (0.0FTE, Days, On-Call) to Providence St. Patrick Hospital in Missoula, Montana. In this position you will:

  • Manage anticoagulation therapy protocols as a part of a multidisciplinary team.

  • Perform point of care testing as appropriate for therapeutic monitoring.

  • Responsible for the coordination and quality of care of SPH Anticoagulation Clinic patients.

  • Evaluate INR responses identify risk factors, and facilitate brief interruptions in therapy across various disciplines.

  • Direct patient care and telephonic management of anticoagulation therapy for patients enrolled in the SPH Anticoagulation Clinic.

  • Collaboration with referring provider to ensure appropriate and safe anticoagulation therapy management.

  • Review, interpret, and evaluate medication and other factors that affect INR management to identify, prevent and resolve potential or actual problems.

  • Recommend and/or implement care plan within protocol or after consultation with referring provider.

  • Responsible for monitoring and tracking all patients referred to the Anticoagulation Clinic.

  • Responsible for POCT (Point of Care Testing) and dose adjustments as outlined in the Anticoagulation Clinic Protocol.

  • Identify, prevent and resolve potential or actual medication related interactions with warfarin. Recommends and/or implements care plan once problem is identified and referring provider has been consulted/notified.

  • Review medical history, clinical and patient information, INR results, and vital signs if needed and patient compliance and using clinical judgment determine treatment doses of anticoagulant therapy for patients. To advise each patient INR results, anticoagulant dose and determine date for next Anticoagulation Clinic appointment.

  • Ensure prompt attention to urgent and abnormal results and responsive mediation to unpredictable circumstances such as extreme INR levels or patients bleeding.

  • Must be able to evaluate patient history and develop a bridging plan to present to referring provider.

  • Notify referring providers of any abnormal finding (including vital signs), change in condition and response to treatment, or if INR falls outside of range as outlined in protocol.

  • Organize and prioritizes work to provide patient care in a safe environment to maximize health in the continuum of care and protect the patient.

  • Participate in the care of patients and is accountable for working with a team of providers by following proper channels of communication, anticipating and consistently handling daily and routine problems and recognizing complex issues that need referral to provider.

  • Understand and provide competent care to a diverse population. Uses knowledge of cultural differences to provide nursing care that is sensitive to cultural values and beliefs.

  • Provide patient care in a professional manner. Demonstrates commitment to customer service.

  • Provide anticoagulation education to new patients and ongoing education to all patients.

  • Practice health care team building and leadership skills. Applies leadership skills to facilitate effective team functioning & operations of the department. Role models, & facilitates change within the health care team respond to health care and organizational needs. Directs, delegates and mentors quality of care delivered by the health care team and brings deviations to the attention of staff involved and the Team Leader. Mentors new staff and assists with orientation and staff development needs.

  • Participate in ongoing continuing education activities & shares knowledge with other health care team members.

  • Access, evaluate and use of computer bases information and practice management for decision support, provision of care, continuous quality improvement and professional development. Provides input to the development of the information systems that impact provision of patient care.

  • Responsible for Monthly quality control on all POCT monitors in Anticoagulation Clinic.

  • Participate in required lab proficiencies at least once quarterly, and skills testing yearly.

  • Responsible for in basket management for all incoming calls. Returning calls and documentation in Epic.

  • Assure all orders and referrals are current and accurate before seeing patients.

  • Complete medication refills per IHI med refill protocol and use ACC smart phrase with each refill encounter in EPIC.

  • Administer anticoagulation medications (i.e.: Fragmin, Coumadin, Lovenox) as ordered by the providers

  • Ability to talk with patients via phone to explain dose adjustments, changes, etc.

  • Facilitate discussion between patient and referring provider or other providers as needed within the multidisciplinary team.

  • Actively keep patients and families informed of care and anticipated treatments.

  • Actively participate in meetings and committees to improve individual and departmental organizational performance.

  • Track and report to billing personnel orders for monthly supplies ordered for clinic.

  • Assist with front office staff with work queues and patient charges when necessary.

  • Send prescription refills through EPIC and manage all Warfarin/Coumadin refills for the clinic as per protocol.

  • Participate in meetings, committees and continuing education to improve both individual and departmental organizational performance. Takes initiative to identify areas for improvement and follows through to completion.

  • Report and document any unusual patient or administrative occurrence that may have an effect on the patient’s and/or staff’s well being.

  • Use communication skills to answer phones, take complete and accurate phone messages and documents all calls in EPIC. Routes calls to appropriate pools as needed. Closes encounters when telephone call complete.

  • Willing to assist and accept assistance from appropriate team and staff members.

  • Attend educational offerings as appropriate.

  • Document all clinical information supporting assigned tasks accurately and completely in a timely manner.

  • Document consumption of supplies and makes charges appropriately.

  • Document forms clearly, legibly and uses only approved abbreviation.

  • Maintain current records on equipment with protocols for checking such as, BCA PC, AED, 02 tank, thermometer, crash cart, etc.

  • Communicate appropriately and professionally in an honest and respectful manner with colleagues, supervisors, physicians, peers, patients, etc.

  • Communicate clearly and appropriately using age specific standards with patients and families of patients.

  • Flexible with staffing needs to the outpatient setting, physicians, patients and co-workers.

  • Follow through on problems that may compromise effective performance.

  • Respond to needs for assistance and service from co-workers, patients and referring providers, in a timely manner.

Qualifications:

Required qualifications for this position include:

  • Graduate of accredited RN nursing program.

  • Current unrestricted RN Licensure in the state of Montana as needed for Warfarin management.

  • Basic Life Support (BLS) card must be from the following authorized governing bodies: American Heart Association (AHA) - (BLS Provider), American Red Cross (ARC) - (BLS for Health Care Provider), or Military Training Network (MTN) – (Healthcare Provider) upon hire.

  • Two (2) years clinical RN experience, resulting in excellent clinical skills and judgment.

Preferred qualifications for this position include:

  • Bachelor's Degree in Nursing.

  • Certification in CACP, CVRN or Med/Surg.

  • Recent Anticoagulation Management, Cardiology or Med/Surg experience.

  • Decision support experience, Electronic Medical record (EMR) experience.
