Providence Health & Services Palliative Care Coordinator in Missoula, Montana
Providence is calling a Palliative Care Coordinator (Palliative Care, Day Shift, 0.5 FTE/20 hours per week) to Providence Medical Group in Missoula, Montana. We are a community of caregivers delivering every day on our Mission to provide compassionate care that is accessible for all – especially those who are poor and vulnerable.
In this position you will:
Assist and educate patients and families in making informed choices in the use and completion of an Advance Directive document.
Structure the development of a comprehensive educational program for SPH staff, physicians, and the community at large regarding the mission and philosophy of the ACP program for St Patrick Hospital.
Coordinate the Palliative Care Services (PCS) clinical services.
Provide clinical support, education and development, and case management support for patients in the hospital.
Provide timely response to referrals, educate/mentor the interdisciplinary care team and other clinicians in evidence-based palliative care, and assist patients/families with post-hospital discharge care planning.
Required qualifications for this position include:
Bachelor’s Degree in Nursing
Current licensure as Registered Nurse in the state of Montana
Incumbent will obtain Case Management certification and/or Certification of Hospice Palliative Care Nursing within three (3) years of employment
Minimum of three (3) years hospital experience
Preferred qualifications for this position include:
Master’s degree in health care related field
Hospice and/or Home Care experience
Experience in staff development, education, and precepting nurses and other clinicians
