Providence Behavioral Health Consultant (LCSW) in Missoula, Montana
Providence is calling a Behavioral Health Consultant (LCSW) (Grant Creek Family Practice Clinic, Day Shift, 1.0 FTE) to Providence Medical Group in Missoula, Montana. In this position you will:
Work’s effectively in a patient-centered, integrated model in a variety of clinical settings.
Participates in integrated care team meetings to coordinate care and develop plans of care.
Determines mental health diagnosis when needed according to current version of Diagnostic and Statistical Manual of Mental Disorders.
Construct, document, and execute robust treatment plans using the biopsychosocial model and evidence based interventions.
Utilize evidence-based clinical tools for tracking clinical outcomes (PHQ-9, GAD-7, AUDIT-C, etc).
Provides brief intervention and/or brief treatment for substance abuse as indicated, adhering to evidence-based techniques. Interface with community substance abuse treatment centers to coordinate care as appropriate (SBIRT).
Engages in trainings to learn and improve clinical skills as appropriate.
Schedule 30-45 minute appointments for follow-up visits, utilize telephone encounters and MyChart secured messaging as adjunct follow-up to maximize the number of patients followed within IBH.
Communicates treatment progress and recommendations to the patient’s PCP and other members of the care team.
Provide brief behavioral interventions using evidence-based techniques such as behavioral activation, problem solving treatment, motivational interviewing, and other treatments as appropriate.
Provide or facilitate in-clinic or outside referrals to evidence-based psychological treatments as clinically indicated.
Develop and complete relapse prevention self-management plan with patients who have achieved their treatment goals and are soon to be discharged from the caseload.
Provides clinical assessment with knowledge of biological, psychological, and social-environmental assessment strategies relevant to individuals and systems:
Conduct mental status examination
Conduct crisis assessment
Conduct assessment for transition to other level of care
Assess education needs of patient
Implement treatment plan
Systematically track treatment response and monitor patients (in person or by telephone) for changes in clinical symptoms and treatment side effects or complications.
Support psychotropic medication management as prescribed by medical providers, focusing on treatment adherence monitoring, side effects, and effectiveness of treatment.
Track patient follow up and clinical outcomes using a registry. Registry functions can be accomplished through an EHR build, on a spreadsheet used in conjunction with an EHR, or can be built into a stand-alone clinical management tracking system that may or may not be linked to an EHR.
Document patient progress and treatment recommendations in an EHR and other required systems so as to be shared with medical providers, psychiatric consultants, and other treating providers in a timely manner.
Facilitate treatment plan changes for patients who are not improving as expected in consultation with the medical provider and the psychiatric consultant and who may need more intensive or more specialized mental health care.

Train and teach effective behavioral health interventions and the primary behavioral health model to medical practitioners and other clinic staff.
Contribute to the optimum mental health of the team by providing consultation, debriefs and coping skills and education
Plans for psychiatric emergencies:
Be available to step out of sessions as needed for crisis support
Performs risk assessment for dangerous or destructive behaviors
Assists patients in writing individual crisis plans
Maintains current skills in managing assaultive and suicidal behaviors
Required qualifications for this position include:
Master's Degree in Social Work
Montana Licensed Clinical Social Worker, or license eligible in the State of Montana
3 years experience with adult populations and familiarity with broad range of psychiatric disorders
Preferred qualifications for this position include:
It is highly preferred that the LCSW achieve his/her OSW-C certification within 5 years of starting this position
Prior experience in integrated behavioral health
Thorough knowledge of integrated care principles and operations
